Driving Systems Change to Build Healthy Communities

Transform Health plays a vital role in California’s CalAIM ecosystem —functioning as a connector across Medi-Cal, Managed Care Plans (MCPs), provider networks, and community-based organizations. Their Collaborative Planning and Implementation (CPI) sessions, policy insights, and collaborative approach reinforce a high-functioning environment where implementation meets lived reality. MISI is honored to engage with partners who bring this level of structure and leadership.​

I found Transform Health to be thoughtful, intentional, and enlightening in the support that they provided, always considering what would be the best fit for our program. They were proactive in identifying solutions for us and they offered high-quality technical assistance. The deep experience of their team was hugely valuable to me, and I could always rely on the quality of their work and professionalism. Transform Health not only provided support that was critical during Open Enrollment, but their help allowed us to improve the NH Navigator program for the future, benefiting all consumers in New Hampshire.

Transform Health did a tremendous job helping our program plan and execute challenging conversations in the health care policy and sustainability space. We truly could not have achieved the outcomes we did without their support, insight, and guidance. Because of their expertise, we are able to move forward with our next set of goals, and we plan on working side by side with Transform Health to make this happen.

The Transform Health team helped us see themes and best practices across states and led us through several sensemaking sessions to identify strategic opportunities for our partners working to improve how people navigate health coverage in our state.

Transform Health has created a space for grantees to informally connect, share outreach and enrollment best practices, and discuss program objectives through their peer-to-peer roundtables. Their staff are great facilitators and clear subject matter experts. In addition to the roundtables, I worked with Transform Health to create a case study of OAF's Navigator program. That study has been and will continue to be a resource when communication the value and impact of our work.

I want to thank Transform Health for all their hard work and dedication. The team of supportive and knowledgeable staff has provided us the information to grow our programs and understand the changing landscape. It has been an amazing journey from Whole Person Care to the CalAIM program with the impact we have made on so many people’s lives.
