Calling All Enrollment Assisters, Patient Navigators & Community Health Workers

This has been an unprecedented year in health care enrollment and patient navigation, with dramatic highs and lows but, at the end of the day, consumers still need access to affordable, quality health care. Transform Health has teamed up with Families USA to offer an enrollment- focused track at this year’s Health Action 2019 conference. These workshops come with the recognition that there is a need to transform the broader health system by further addressing social determinants of health, building solutions to housing that also address health, and implementing the parts of the Affordable Care Act that are still the law of the land. We also recognize that, across the country, Community Health Worker models are on the rise, consumer assistance is still a cornerstone, and patient navigation is front and center to helping people access appropriate levels of care at the right time to stay healthy.

Enrollment assisters, patient navigators, community health workers, outreach entities, and others who support this work, such as Young Invincibles, Community Catalyst and the National Health Law Program’s In The Loop, the Center on Budget and Policy Priorities’ Health Reform Beyond the Basics, and the National Association of Community Health Centers, are important stakeholders for the upcoming 2019 Health Action conference.

As the trusted messengers for our communities, we recognize that none of this change can happen if people are not enrolled in and retain coverage. In an ever-changing plan, provider, hospital system and formulary landscape, consumer assistance and patient navigation remains front and center year-round.

In years past, the workforce shepherding consumers into the health care delivery system have been mission critical, and this year is no different. Health Action 2019 will be a great opportunity for community health workers, enrollment and patient navigation entities to come together and share lessons learned, best practices, insights, and ideas for moving forward to improve the delivery of health care services for low-income and other vulnerable populations.

While the Health Action conference will be chock full of workshops covering a range of topics, from social determinants of health to legislative updates, we are highlighting and sharing a couple of the sessions most relevant for people and organizations working to enroll people in coverage and help them navigate the health care system once enrolled. We are paying particular attention this year to models that are adapting in this landscape of change. Here is a brief rundown of the topics that we are partnering to cover:

Outreach, Enrollment, and Patient Navigation in Today’s Climate

This workshop will go over recent and upcoming changes to the health care landscape and how they affect the consumers and patients who need continued help navigating coverage and care. Panelists will discuss new efforts they’re implementing to continue meeting essential needs of their communities.

Consumer Assistance is Still Mission Critical: Where We’ve Been, Where We’re Going

Open enrollment is once each year, but people still need help year-round understanding health care in the changing landscape. This panel will explore where enrollment and patient navigation are now and where we need to go.
