At the end of each year we come upon the next open enrollment period for people to decide on their health insurance coverage options. Transform Health, a women and minority-owned national health care consulting firm, in partnership with Whitman-Walker Health, a Federally Qualified Health Center in Washington, DC, came together to highlight the critical role that navigation has in making these time-sensitive decisions in this white paper. Navigators ensure that individuals and families who need the most support enroll in coverage, stay insured, overcome barriers to accessing care, and connect to community resources that address the social conditions impacting their health...…

When Jaime suffered from a major stroke nearly two years ago, his world turned upside down. A professional chef and rising star in the local food scene, Jaime was the creative force behind a series of local pop-up events and farm-to-fork dinners. He thrived on sharing ideas and connecting with others, whether it was working on film projects or volunteering in the community.
Then everything changed. Jaime experienced an ischemic stroke, meaning that a clot had blocked ..…

Last year was a whirlwind for the thirty-five-year-old Palmetto Project in South Carolina, a statewide nonprofit whose mission is to identify innovative strategies to address the social and economic challenges in the state and put them into action. The nonprofit is no stranger to solving big problems. When the Affordable Care Act (ACA) passed in 2010 and shortly thereafter in 2013 the Navigator grants were announced, the Palmetto Project was front and center in a state whose politicians were largely against the ACA, and still has not expanded Medicaid. The Palmetto Project staff were serving consumers who desperately needed and wanted health coverage, convening state partners, and persisted in leading the way in sharing and implementing best practices …..
While a few state-based marketplaces are still finishing up the most recent open enrollment period, we already have a few key lessons we can take away and some key achievements that must be lauded, despite the significant cuts to outreach and Navigators during this Administration. Past years have taught us that …

This has been an unprecedented year in health care enrollment and patient navigation, with dramatic highs and lows but, at the end of the day, consumers still need access to affordable, quality health care. Transform Health has teamed up with Families USA to offer an enrollment- focused track at this year’s Health Action 2019 conference. These workshops come with the recognition …
We are only hours away from the beginning of the sixth open enrollment period for states using healthcare.gov, and this year is not without its challenges. A broad array of stakeholders—community partners, assisters, community health workers, health centers, friends and family of people in need of health care, and others—all have roles to play to make sure people understand their coverage options and can make informed…

Fall is quickly becoming open enrollment season for many. For those are not eligible for Medicare and are interested in health insurance through the marketplace made available through the Affordable Care Act (ACA), open enrollment generally runs November 1 through December 15. States with their own marketplace can extend these deadlines. Open enrollment for people with Medicare runs October 15 through December 7…..

Like the rest of the country, budgets and support for in-person outreach and enrollment convenings are limited due to funding cuts for enrollment navigation. But the conference made it crystal clear that there is appetite for more training, information, and opportunities to learn from one another and that there is continued pressing need from consumers. Generous sponsorships…
Homelessness, and being on the verge of homelessness, is both a housing and health care problem on the rise nationwide. According to the most recent national homeless census, on any given night, over half a million people are experiencing homelessness, with more than a third sleeping outside in cars, parks, sidewalks, and on the streets.
Despite growing public awareness that housing and health outcomes are interconnected, services for this population are usually disconnected and provided in times of crisis. The result — a seemingly endless cycle of continued homelessness, poor health outcomes, and high health care and government costs.
It’s no surprise that cities and counties across the nation are grappling with how to support ….
