Anthony joins Transform Health as the Director of Transformation in California’s Central Valley. For the past decade he has served his community through direct services in housing navigation and case management, with expertise in trauma-informed care. Most recently, Anthony led implementation efforts under the California Advancing and Innovating Medi-Cal (CalAIM) at a non-profit in the heart of the Central Valley that provided access to housing navigation, enhanced care management and supportive services to community members experiencing homelessness, living with substance abuse disorders, and severe mental illness.

At Transform Health, Anthony facilitates regional relationships in the Central Valley to support our work in the region, including leading the stakeholder engagement efforts in our portfolio of work. Anthony’s expertise in housing navigation and case management are key drivers in supporting our health systems’ change work.

Anthony has helped numerous nonprofits and government institutions build needed infrastructure to deliver services leading to impactful changes from the ground up. His time serving the unhoused community has provided him with hands on experience in implementing a pilot program with Child Welfare Services to reunify families and support youth. His experience in designing programming includes project planning, strategic planning, building bridges in community and implementing trauma informed care techniques to ensure clientele receive quality care coordination.

Anthony has a master’s degree in business administration from Western Governors University and a bachelor’s degree in history from CSU Fresno. In his free time, Anthony can be found watching movies with his wife at the local cinemas, taking long walks in the countryside, or watching the best team in all of sports – the San Francisco 49ers, of course!
